Abstract

Workplace accidents remain a significant issue in the manufacturing industry because they impact worker safety and the continuity of company operations. This study aims to analyze the factors causing workplace accidents and mitigation efforts among employees of PT Isoku Protection Sanbrilliant. The study used a qualitative approach with primary data obtained through in-depth interviews, observation, and documentation. Research informants included company leaders, production managers, and occupational safety and health (K3) officers. Data analysis was conducted using triangulation techniques to ensure data validity. The results show that human error is the main cause of workplace accidents, influenced by physical and mental fatigue due to workload, overtime, and production target pressure. Equipment and machinery factors are not dominant due to routine inspections. However, non-compliance with SOPs and the use of personal protective equipment (PPE) still contribute to workplace accidents. Preventive measures are carried out through K3 training, equipment inspections, and toolbox meetings, b ut ongoing supervision and management of work fatigue are needed.

Keywords: workplace accidents, occupational safety and health (OSH), human error, manufacturing industry.
